Referral information

Please note that our ability to offer a place will be based upon:

• Placement availability
• Funding availability by the placing authority
• A suitably skilled and experienced carer capable, able, and available to meet the child's identified needs
• NEWFOCAS' ability and capacity to meet the child's identified needs
• An initial two-way risk assessment relating to the referred child, and any other children already placed or living with the identified carer. This risk assessment will include any potential risk to the carers.

To make a child referral please complete the following forms:

• Referral Stage 1 - Child / Young Person
• Referral Stage 2 - Child / Young Person
• Risk Assessment - Child / Young Person

To make a Parents and Baby referral please complete the following forms:

• Referral Stage 1 - Parent & Baby
• Referral Stage 2 - Parent & Baby
• Risk Assessment - Parent & Baby
